Building Community Health Together

The Greater Hamilton Health Network (GHHN) is one of the first Ontario Health Teams across the province, with a purpose of transforming healthcare in partnership with patients, families, care partners, primary care, local organizations and the community.

A collaboration of local health and social service partners, the GHHN includes representation from more than 70 organizations.  We are a dedicated group of professionals, organizations and people with lived experience working to co-design a patient centred health system, grounded in engagement, health equity and the local needs of the communities we serve across Hamilton, Haldimand, and Niagara Northwest.

 

The Greater Hamilton Health Network is supported by funding from the Government of Ontario.

Patient Booking

The Greater Hamilton Health Network is helping connect people to medical providers in Hamilton, Haldimand and Niagara Northwest. This service will allow patients and caregivers to book an appointment with their medical provider online.

As summer draws to a close, I wanted to take a moment to reflect on the journey we’ve been on and to acknowledge the challenges we’re facing together. The recent months have been particularly demanding, with health issues in our community placing significant stress on so many. It’s been a tough period, and I want to be honest about the difficulties we’re encountering across sectors in so many areas. The complexity of the problems we’re addressing requires more than just our individual efforts—it necessitates a collaborative and integrated approach. As we move forward, our success hinges on the strength of our partnerships and our ability to work together in new and innovative ways.

Building Health Equity in the Greater Hamilton Area

In the fall of 2020, the Greater Hamilton Health Network began the important work of implementing their vision to provide an equitable and seamless continuum of care that actively improves population health, while meeting the individual needs of the community.

As part of this work, the GHHN recognized and identified the need to engage the voices of communities who face systemic discrimination or marginalization to understand how this impacts access to universal and accessible health care, knowing that inequity drives poor health.

We are pleased to share our community driven Health Equity Report that shares the voices of our communities and our bold vision for an equitable and culturally safe healthcare system that is patient centered.
